  5. Track closure. Someone needs to confirm the action was actually completed, not just marked as done.

  • No clear ownership for corrective actions. Findings without an assigned owner tend to get forgotten.
  • Inspecting only what is easy to check. Genuine hazards are sometimes the ones that take more effort to assess properly.
